In David (the closest larger city to Boquete, less than an hour away by car and where you would go for medical care for more serious conditions), there are many doctors who speak English.

There are local websites originating here in Boquete that have a list of which doctors speak English, some English, or no English in Boquete and David. There are plenty who speak full English, so if you do not speak Spanish, this is not an issue for you.

But once you are already with the doctor, the communication is usually very good even if you do not speak Spanish.
Most of the doctors in Boquete went to medical schools in the US so they have learned enough English.

One positive aspect for me, is I can go to a medical lab in Boquete and order what test I want, with no doctor visit necessary. So, I get my lab tests in Boquete, and then take my results to my doctor in David. This saves time and puts me in control of what I want to check on.
Since I speak Spanish, it is not necessary that my doctor speak English, but it is far more reassuring for me to be able to speak to my doctor in my native language.
