Unfortunately, there are no English-speaking medical doctors in San Juan del Sur. Medical services are available at the local clinic but they are limited based on the unavailability of medical equipment, e.g. X-ray machine, EKG, scans, ultrasound and the like.

My family and I have used Dr; Rodriquez at the clinic who makes house calls at a reasonable rate of $10-15 dollars. There is a full service hospital in Rivas that has treated ex-pats for trauma and illness successfully. In Managua we go the Vivian Pellas hospital for tests and if necessary, surgery. All of the specialized equipment necessary to do diagnosis and treat injury and disease if available there at approximately 40% of the cost of the same service in the US. Vivian Pellas has a modified health care plan called "Club de Salud" which provides services at a discount, in some cases up to 80%. A monthly fee is paid in the $35 range. Details are available for this service through the hospital.
Many specialists maintain their office at Vivian Pellas hospital. For example, I go to the lab for a semi annual PSA blood test and generally a full panel of blood test to check on my cholesterol and others at a cost of $97. If necessary, I will see urologist for an exam, not because I have a problem, but to prevent and detect. Usually with the expense of traveling to Managua, lunch and fees, I spend $200. This is far less than I would spend flying to California to see my doctor to do the same thing at a cost of at least $2,000. including airfare.
The hospital in Rivas is 30 minutes away by car or $2 (that's right two bucks) for a taxi. Vivian Pellas is 1 hour and 45 minutes from San Juan del Sur by car.

I am pretty sure though that there are a few physicians who could express themselves in a little English to the extent that an American can understand but if you really want a physician who studied abroad, or particularly someone who studied in the US and can speak English very well, I would recommend going to a physician in Managua (2 hours away by car).

Even if the physicians in San Juan del Sur do not speak English very well you would still get good care. I’m sure that nowadays, with so many more expats living in San Juan Del Sur, even local doctors must be learning how to speak English. I’m sure they experience running into Americans who need medical attention or Americans who see them for a checkup and they can communicate.

I ended up being seen by two doctors in San Juan Del Sur, both of whom spoke English. They were excellent. When I checked back with my doctor in Toronto, he told me that the type of care that the doctors here in San Juan Del Sur provided and the prescriptions that they wrote for me...

In San Juan Del Sur, you get very good, affordable, and very quick care. My whole bill for the incident I described was about US $65. This included several liters of fluids that they got me and the doctor’s fee, which included coming back and seeing me after a couple of days to check that my reaction to the poison had gone down.
In the end, I went to Hospital Metropolitano Vivian Pellas in Managua, which is about an hour away from San Juan Del Sur and is a first class hospital. The care they gave me was amazing and fast. I was seen immediately, as soon as I walked in the Emergency Room. The quality of care was excellent.

There must be at least one English-speaking doctor in San Juan Del Sur since there has been a lot of progress here lately.
There are a lot of Americans and Canadians coming over here now to live and open up businesses, many of whom don’t speak Spanish, so I would assume that there would be doctors who could talk with them and treat them, while speaking English.

There are no English-speaking doctors in San Juan Del Sur. The doctors here in San Juan Del Sur can handle minor cases such as a cold, flu, or a rash. If you have these types of conditions, you could go to a pharmacy and consult with a doctor. You don’t need to set an appointment to do that but they will give you the remedy that you need.
If you go surfing and you get a fin to the forehead and you need stitches, you can go to the Centro de Salud,...

If you only need simple stitches, I highly recommend going to Centro de Salud. If you break a bone, you can go and get an x-ray in Rivas (a nearby city), which would cost around US $25. For more complicated cases, you may go to Managua, which is 2 hours away from San Juan Del Sur by car, and you can either go to the military hospital or to Vivian Pellas Hospital, where you will find English-speaking doctors that provide American standard service for 1/3 of the price in the US.
There are some insurance programs with the hospitals here in Nicaragua. You could also get a gold membership card, which would entitle you to 50% off of any of your hospital bills. Even if you stay in the ICU for a week, the cost is still somewhat reasonable. You could manage the amount especially if you have the gold membership. The hospitals bills here in Nicaragua will not make you go bankrupt. It is like less than a hotel stay.
