There are no fresh water activities in San Juan Del Sur. Most of the activities are at the ocean.
If you want to go to a river, the closest you can go to is Rio San Juan, which is south of San Juan Del Sur on the border of Costa Rica. You can also go to Granada Lake which is about an hour by car from San Juan Del Sur if you want to do some fresh water activities.

We have Lake Nicaragua right behind San Juan Del Sur. Lake Nicaragua is one of the biggest lakes in the world and the second biggest lake in Latin America. Off of San Juan Del Sur, there are some little rivers, but there is nothing big enough for river rafting. Also, there are definitely no cascades because the land behind San Juan Del Sur is relatively flat so there is a very slim chance of finding waterfalls in San Juan Del Sur.

It is more like a river in the sense that it is more like a canal. In most cases, estuaries start where the ocean and the sand meet and then go inland.
